Safety Checklist

  • Confined space identified and properly labeled
  • Entry permit completed if required
  • Air monitoring conducted before entry and continuously during work
  • Ventilation in place to maintain safe air quality
  • Attendant assigned outside the space at all times
  • Communication established between entrant and attendant
  • Rescue plan in place and understood by the crew
  • Proper PPE and equipment available and used
  • Lockout/tagout applied where necessary
  • Unauthorized entry prevented

Crew Talking Points

  • Is this a permit-required confined space?
  • What are the atmospheric hazards we need to monitor?
  • Who is the attendant and how will we communicate?
  • What is the rescue plan if something goes wrong?
  • Are we continuously monitoring air quality?

Stop Work If

  • Air monitoring is not being conducted or shows unsafe levels
  • Ventilation is not working properly
  • No attendant is present outside the space
  • Communication is lost between entrant and attendant
  • The rescue plan is unclear or not in place
