Maintaining Entry Logs Confined Spaces Toolbox Talk

Toolbox talk on confined space entry logs, entrant tracking, permits, attendants, timing, and emergency accountability.

Confined space entry logs help the crew know exactly who is inside a space, when they entered, and when they exited. In tanks, vaults, pits, manholes, crawl spaces, vessels, and utility spaces, missing or inaccurate entry records can delay evacuation, confuse rescue efforts, and put workers at risk.

This talk focuses on maintaining accurate entry logs during confined space work. The goal is to make sure the attendant tracks every entrant, keeps the permit information current, and confirms all workers are accounted for before, during, and after entry.

Why This Matters

  • Entry logs provide accountability for every worker inside the confined space.
  • Accurate times help monitor exposure, fatigue, heat stress, and work duration.
  • Logs help attendants, supervisors, and rescue teams know who may need help during an emergency.
  • Clear records prevent unauthorized workers from entering the space.
  • Updated logs support permit compliance and help confirm the space is clear before closing the permit.

Common Hazards

  • Workers entering or exiting without notifying the attendant.
  • Using a permit or log that is incomplete, hard to read, outdated, or not kept at the entry point.
  • Failing to record entry times, exit times, worker names, air readings, or shift changes as required.
  • Allowing unauthorized workers, visitors, or other trades to enter without being added to the log.
  • Losing track of entrants during breaks, tool runs, task changes, or multiple entry points.
  • Letting the attendant become distracted and miss a worker entering or leaving.
  • Working in a large or connected space where workers can move out of sight and exit through a different opening.

Safety Checklist

Before Work Begins

  • Review the confined space permit and confirm entry log requirements.
  • Identify all authorized entrants, the attendant, entry supervisor, and rescue contacts.
  • Set up the entry log at the controlled entry point where the attendant can use it.
  • Confirm how names, entry times, exit times, air readings, and re-entry checks will be recorded.
  • Control all access points so no one can enter without the attendant knowing.
  • Make sure workers understand they must check in and check out every time.

During Work

  • Record each entrant before they enter the confined space.
  • Log exit times immediately when workers leave the space.
  • Update the log after breaks, shift changes, re-entry testing, changes in work scope, or changes in personnel.
  • Keep communication open so the attendant knows where entrants are and how they are doing.
  • Do not allow anyone not listed or authorized on the permit to enter the space.
  • Confirm all workers have exited before closing the permit or removing entry controls.

Crew Talking Points

  • Where will the entry log be kept during this confined space entry?
  • Who is responsible for recording names, entry times, exit times, and permit updates?
  • How will we control additional access points or connected areas?
  • What is the process for breaks, re-entry, shift changes, and new workers joining the task?
  • Does anyone have questions or concerns about entry tracking, authorization, communication, or accountability?

Stop Work If

  • The entry log is missing, incomplete, outdated, or not available at the entry point.
  • The attendant cannot confirm who is inside the confined space.
  • A worker enters or exits without being recorded.
  • Unauthorized workers are near the entry point or attempting to enter the space.
  • Multiple openings, shift changes, or task changes make entrant tracking unclear.

Final Reminder

An entry log is part of the rescue plan. Track every worker, update the log immediately, and never continue if the crew cannot account for who is inside.
